St. John's wort is the single most important herb-drug interaction in the supplement world, and it is worth being precise about why. It causes trouble two completely different ways. First, it speeds up the liver and gut machinery that clears many prescription drugs, quietly lowering their blood levels until they stop working — that is how it can make birth control fail or push a transplant toward rejection. Second, it raises serotonin, so combining it with antidepressants and certain migraine drugs can stack too much serotonin at once. The practical effect is counterintuitive: the danger is usually not that the other drug becomes too strong, but that it becomes too weak. This is not immediate — enzyme induction builds over one to two weeks and takes a similar time to wear off after you stop, which makes the effect easy to miss.
2. It raises serotonin
Separately, St. John's wort has serotonergic activity of its own. Combined with drugs that also raise serotonin — SSRIs, SNRIs, MAOIs, triptans for migraine, tramadol, and the supplement 5-HTP — it can contribute to serotonin syndrome, a potentially serious build-up of serotonin. Hormonal birth control. Warfarin (a blood thinner). St. John's wort can reduce warfarin's blood-thinning effect, meaning your blood may clot more easily than your dose is meant to allow — raising the risk of a clot. Because warfarin is dosed to a narrow target measured by INR blood tests, any herb that shifts it is a real concern.
Immunosuppressants (cyclosporine, tacrolimus). These prevent transplant rejection, and their levels have to stay in a tight window. St. John's wort can drop them enough to trigger organ rejection — one of the clearest and most serious documented interactions. Transplant recipients should avoid it entirely.
Some HIV and hepatitis C antivirals. St. John's wort can lower levels of certain protease inhibitors and other antivirals, risking treatment failure and resistance. The FDA has specifically warned against combining it with the HIV drug indinavir.
Certain heart drugs (digoxin), some cancer therapies (e.g. imatinib, irinotecan), some statins, and some anti-seizure medications. All can have their levels reduced. With narrow-margin drugs like digoxin and oral chemotherapy agents, that matters a great deal.
Antidepressants and other serotonergic drugs. Here the issue is additive serotonin, not reduced levels — the serotonin-syndrome risk described above.

What to actually do
